On May 25, 1969, a pallet of ammunition caught fire at a supply and storage area near Quang Tri, South Vietnam. The facility was maintained by elements of the 3rd Marine Division. As nearby Marines rushed to put out the blaze, the pallet took a direct hit from an incoming mortar round. The resulting explosion killed thirteen Marines and wounded nineteen more. Two others could not be located after the blast and were declared missing in action.

Private Monek Weitz joined the U.S. Marine Corps from Massachusetts and was a member of A Battery, 1st Battalion, 12th Marine Regiment, 3rd Marine Division. He was among the Marines who rushed to extinguish the burning ammunition pallet, and was one of the two men who could not be found after it exploded. It is believed that the force of the blast made recovery of his remains impossible. Today, Private Weitz is memorialized on the Courts of the Missing at the National Memorial Cemetery of the Pacific.
