On May 1, 1967, a CH-46A Sea Knight (bureau number 150268) carrying seventeen on board that included crew members and passengers, was on a medical evacuation flight to the hospital ship, the USS Sanctuary (AH 17), when it crashed for unknown reasons and into the sea off the coast of South Vietnam. The aircraft wreckage quickly sank, but search and rescue efforts rescued nine survivors and also recovered the remains of two others killed in the crash. However, one crew member and five passengers were not recovered.

Lance Corporal Carl Arthur Smith, who entered the U.S. Marine Corps from New York, served in Company B, 1st Shore Party Battalion, 1st Marine Division. He was a passenger on this helicopter when it crashed on May 1, 1967. He could not be recovered and remains unaccounted-for. Today, Lance Corporal Smith is memorialized on the Courts of the Missing at the National Memorial Cemetery of the Pacific.
