On April 1, 1967, an F-4C Phantom II (tail number 64-0746) with two crew members took off as the lead aircraft in a flight of three on a strike mission against a Viet Cong storage area. The F-4 made three passes over the target, but on its third pass, it failed to pull up from its dive and crashed near the target area. The other aircraft on the mission lost radio contact with both crew members, and no parachutes were seen to emerge from the aircraft prior to the crash. Attempts to locate the crew members following the incident were unsuccessful. 

Captain George Henry Jourdenais entered the U.S. Air Force from Rhode Island and served in the 557th Tactical Fighter Squadron. He was the aircraft commander aboard the F-4 when it crashed and was lost along with the aircraft. His remains were not recovered. Following the incident, the Air Force promoted Captain Jourdenais to the rank of Major (Maj). Today, Major Jourdenais is memorialized on the Courts of the Missing at the National Memorial Cemetery of the Pacific.
