On September 19, 1966, an F-4B Phantom II (bureau number 152315) carrying two crew members performed a catapult launch from the deck of the USS Constellation (CVA 64), stationed in the Gulf of Tonkin. When it was about one mile forward of the ship, the Phantom crashed into the water and exploded. A search of the crash site was conducted, but no sign of either occupant of the Phantom could be located. Both crew members remain unaccounted for.

Lieutenant Frank Monroe Brown Jr., who joined the U.S. Navy from Pennsylvania, was a member of Fighter Squadron 151. He was the pilot of the Phantom when it crashed, and was lost with the aircraft. His remains were not recovered. Today, Lieutenant Brown is memorialized on the Courts of the Missing at the National Memorial Cemetery of the Pacific.
