On June 23, 1966, an F-4B Phantom II (bureau number 152324, call sign "Switchbox 114") with two crew members launched from the aircraft USS Constellation (CVA 64) as the second aircraft in a flight of two. After returning to the carrier and while making his landing approach, the pilot of "Switchbox 114" reported an equipment malfunction and requested to approach the deck on the lead aircraft's wing. "Switchbox 114"  then was observed to pass and go in front of the lead aircraft, descend and roll, and finally impact the water behind the carrier. No ejections were observed. Search efforts recovered only aircraft debris, but did not locate the two crew members.

Lieutenant Junior Grade Lawrence Frederick Nyman entered the U.S. Navy from Washington and was a member of Fighter Squadron 151, embarked aboard the Constellation. He was the pilot of this Phantom when it crashed. His remains were not recovered. Today, Lieutenant Junior Grade Nyman is memorialized on the Courts of the Missing at the National Memorial Cemetery of the Pacific.
