On May 1, 1967, a CH-46A Sea Knight (bureau number 150268) carrying seventeen on board that included crew members and passengers, was on a medical evacuation flight to the hospital ship, the USS Sanctuary (AH 17), when it crashed for unknown reasons and into the sea off the coast of South Vietnam. The aircraft wreckage quickly sank, but search and rescue efforts rescued nine survivors and also recovered the remains of two others killed in the crash. However, one crew member and five passengers were not recovered.

Corporal Roger Conrad Gaughan, who entered the U.S. Marine Corps from Massachusetts, served with Company M, 3rd Battalion, 5th Marines, 1st Marine Division. He was a passenger on this helicopter when it crashed on May 1, 1967. He could not be recovered, and he remains unaccounted-for. Today, Corporal Gaughan is memorialized on the Courts of the Missing at the National Memorial Cemetery of the Pacific.
